Site Form (PDF)  This form needs to be filled out and returned with a $350 deposit to LHMI before final booking can be completed.  The deposit is non-refundable and will be used to cover the setup fee.  If a venue cancels the exhibit, it will forfeit the deposit which will be used to cover costs associated with returning the exhibit to Utica, NY.
